Abstract

저자는 과거 10년간 서울대학교 병원 치과방사선과를 내원한 환자 중 술후성 상악낭의 진
단을 받은 환자 중 117례를 대상으로 병록부, 방사선학적 진단, 조직병리학적 관찰을 통해
다음과 같은 결론을 얻었다.
1. 성별, 연령별 분포는 남자가 82례(70.4%)로 여자보다 많았으며 연령분포는 30대가 40례
(34.2%), 40대가 33례(28.2%)로 30-40대가 가장 많았다.
2. 발생부위는 우측이 62례(53.0%)로 좌측에 비해 약간 많았으며 5례(4.3%)에서는 양측에
서 발생하였다.
3. 처음 상악동 수술시의 연령은 10-45세 였으며 15-35세군이 74례(63.2%)로 가장 많았
다.
4. 환자의 주소는 통증이 68례(58.1%), 팽창이 23례(24.8%), 배농이 9례(7.7%)순으로 나타
났다.
5. 증상발현까지의 기간은 5-50년까지로서 11-15세군이 32례(27.4%), 16-20세군이 31례
(26.5%), 26-30세군이 18례(15.4%)순으로 나타났으며 11-30세군이 전체의 69.3%를 차지했
다.
6. 방사선학적 소견 중 가장 많았던 사항은 단방성이 103례(87.9%), 경계가 명확한 것이
74례(63.2%), 변연이 평활한 것이 79례(68.1%), 병소 중앙부위의 방사선 투과성 증가가 86례
(74.1%), 골경화상이 있는 경우는 74례(63.2%), 상악동 측벽의 변화가 있는 경우가76례
(65.0%)였다.
7. 관련치아의 변화 중 65례(55.5%)에서는 변화가 없었고 치조백선소실이 16례(13.7%), 치
근흡수가 23례(19.7%)로 나타났고 56%는 실활치였다.
8. 감염여부조사에서는 90례 (77.6%)가 비감염성이었으며 변연골경화는 감염시에 상악동
측벽의 변화는 비감염시에 약간 더 증가하였다.

Post operative maxillary cyst may arise after the surgical treatment for maxillary
sinusitis with the symtoms of swelling, pain, and pus discharge in the buccal region. It
is examined by Waters' view, panoramic view and other intraoral radiographs, but quite
variable radiologically.
Most of the cyst is seen round or ovoid shape radiolucency, destruction, expansion
and thinning of the lateral wall or posterior wall, and roots of the adjacent teeth may be
resorbed.
We studied about 117 cases of the post operative maxillary cysts which diagnosed in
department of oral and maxillofacial radiology, Seoul National University Hospital.
We analyzed and obtained following results.
1. These cysts occured more frequently in male than in female and the incidence is
highest in the 4th and 5th decade.
2. Initial radical operation of maxillary sinus were performed mainly between the age
of 10 and 45 years, and about 60% of the patients were 15 to 25 years.
3. Pain and swelling on buccal area, pus discharge, and toothache are most chief
compaints, seven cases were found at routine examination without symtoms.
4. Most of these cysts were unilocular with smooth and well-defined border.
5. The majority of the cysts occurred in the anterolateral wall of maxillary sinus.
6. Dental changes of the lesional area were loss of lamina dura and root resorption,
but about 55% were not changed.
